When billion-dollar systems fail, the cracks aren’t hidden — they’re everywhere.

Last month, three incidents forced me to confront an uncomfortable truth about modern tech security.

Not zero-days.
Not nation-state cyber arsenals.
Not cutting-edge exploits.

Amazon, Google, and the U.S. Department of Justice all failed at the kind of security fundamentals we teach in undergraduate classes. And what shocked me wasn’t the vulnerabilities — it was what these failures revealed about how we actually build systems today.

I’ve spent years shipping production software, reviewing security, arguing with legal teams, fighting deadlines, and cleaning up messes. I thought I’d seen every flavor of failure.

I was wrong.

The 110-Millisecond Lie, We Built Remote Work On

A North Korean operative got hired into Amazon IT.

He passed interviews.
He cleared the process.
He got internal access.

They didn’t catch him with intelligence networks or deep surveillance magic.

They caught him because his keystrokes were 110 milliseconds slow.

He wasn’t in the U.S.
He was remotely controlling a machine from North Korea.
Latency exposed him.

This is the part that makes me uncomfortable. The surveillance state caught the infiltrator. Without that invasive monitoring, he’d still be there. So now we have to ask: Is constant surveillance the only defense against this?

I’ve worked at companies that tracked mouse movements, recorded screens, and measured “productivity metrics.” I hated it then. I still hate it now. But watching Amazon catch an actual threat this way forces an uncomfortable question: Maybe I was wrong?

No. I wasn’t wrong. This is a false choice.

The first question I asked myself was: How did he get hired in the first place?

Then I remembered how hiring actually works at big tech companies:

  • “Remote” means “trust whatever people tell you.”
  • Identity verification means glancing at a PDF.
  • Reference checks rarely happen.
  • Speed matters more than certainty.
We optimized hiring for velocity. So we compensate by surveilling everyone afterward.
We trusted the process. The process never verified trust.

It’s backwards.

And here’s the uncomfortable part nobody talks about: Amazon only caught him because they monitor employees so deeply that it borders on dystopian. Without that monitoring, he’d still be there.

So now we’re left with a brutal question:

Do we accept invasive surveillance to compensate for lazy verification?

My answer: No. We fix the foundation instead of normalizing permanent monitoring.

But most companies won’t.

While Amazon was dealing with its North Korean problem, security researchers discovered something that should terrify anyone using AI in production systems.

Not a bug.
Not an oversight.
A structural flaw in how AI systems behave.

They called the exploit PromptPwnd. I think of it as: “SQL injection with better grammar.”

Developers wired AI agents into CI/CD pipelines and GitHub workflows, granting them real privileges. Then someone opened an issue like this:

And it obeyed.

Google got hit. At least six Fortune 500 companies were vulnerable. The researchers who found it reported it responsibly. Google patched it in four days.

But here’s what keeps me awake:

You cannot truly secure AI against prompt injection today.

Not with better prompts.
Not with sanitization.
Not with layers of “guardrails.”

The problem isn’t implementation.
It’s the architecture.

I’ve shipped AI features. I’ve sat in security meetings about AI risks. And here’s the truth many teams quietly know:

Some systems simply should not have AI anywhere near them.

But companies will keep doing it anyway, because “AI everywhere” sounds innovative on slides.

Until something catastrophic happens.

The DOJ Redaction That Wasn’t

Then came the DOJ “redaction.”

Thousands of sensitive documents.
Names hidden behind thick black bars.
Everything seemingly protected.

Except… you could just highlight the text and copy-paste it out.

The data wasn’t removed.
They just drew rectangles on top.

This isn’t obscure knowledge. Proper digital redaction has existed for over a decade. Every government security training warns against exactly this.

And yet, under pressure and deadlines, the basics fell apart.

I’ve worked with government teams. Procurement is slow. Processes are painful. But even then, I assumed national-level legal documents would use proper tooling.

They didn’t.

The Pattern I Can’t Unsee

Different organizations.
Different tech stacks.
Different stakes.

Same failure pattern:

Speed > Security.
Convenience > Correctness.
Innovation Theater > Basic Competence.

Every security professional already knows:

  • Verify identity before granting access.
  • Never trust user input.
  • Actually, remove sensitive data rather than painting over it.

These aren’t “advanced” ideas.

They’re boring fundamentals.

So why do billion-dollar companies fail at them?

Because nobody gets rewarded for preventing a future disaster. They get rewarded for shipping today.

And honestly?

Most organizations have implicitly accepted occasional catastrophic breaches as “the cost of moving fast.”

Nobody says it out loud. But incentives don’t lie.

What This Changed For Me

I’ve been in rooms where someone says, “We’ll tighten security later.”
I’ve approved risky launches because deadlines mattered.
I’ve watched organizations promise fixes “post-release.”

And I’ve seen what happens after.

So here’s what I now firmly believe:

  • Remote identity verification is a security problem, not an HR process.
  • AI should stay out of privileged workflows — suggestions only, humans approve.
  • Sensitive documents need real technical handling, not cosmetic fixes.
  • If speed requires compromising basic security, it’s the wrong speed.
The cost of doing security right is always lower than the cost of fixing a breach.

Always.

The Real Lesson

We’ve built a hyper-connected, AI-augmented, instant-everything digital world…

…on foundations that are still fundamentally fragile.

That’s not a hacker problem.
That’s a priorities problem.

I still love building systems. I’m still optimistic about technology. But watching these failures clarified something for me:

Right now, the simplest attacks work alarmingly well.

And the sophisticated ones haven’t even really started.
